Seniors and people with disabilities in Ontario now have simple one-phone-call access to Ministry of Health
and Long Term Care funded long-term care services with the launch of Community Care Access Centres across the Province.
Throughout Ontario, 43 Community Care Access Centres (CCACs), which are run by volunteer boards, have been established
to handle both Home Care and Long-Term Care Placement Co-ordination Services. One phone call to the closest CCAC
should give most consumers, families and caregivers the answers they need to access Ministry funded programs.
Community Care Access Centres will:
- provide information on available services and programs
- conduct individual assessments
- determine clients' eligibility for services
- plan a program of care
- arrange for the services to be delivered
CCACs will also provide up-to-date information on other services
including volunteer-based community services (e.g. Meals on Wheels and
friendly visiting), supportive housing, attendant services, children's
treatment centres, retirement residences and other related community services.
CCACs may refer people to ORCA member retirement residences, but a CCAC
referral is not required to access retirement residences. Consumers can
choose retirement residences that meet their own needs and can schedule
a visit and tour almost anytime.
